All Access Health Inc. Provides Service Coordination. The Service Coordinator is responsible for:
Facilitating the Initial Service Plan (ISP) and waiver program eligibility
A service coordinator compiles all documentation needed to establish eligibility, and in conjunction with the individual develops a Service Plan which describes his/her interests, strengths, and goals.
Facilitating that the waiver applicant/participant’s Plan of Protective Oversight (PPO) is completed and supports the service plan
A service coordinator compiles all documentation needed to establish eligibility, and in conjunction with the individual develops a Service Plan which describes his/her interests, strengths, and goals.
Securing initial and annual level of care assessments
A service coordinator compiles all documentation needed to establish eligibility, and in conjunction with the individual develops a Service Plan which describes his/her interests, strengths, and goals.
Responding to participant crises and emergencies
A service coordinator compiles all documentation needed to establish eligibility, and in conjunction with the individual develops a Service Plan which describes his/her interests, strengths, and goals.
Addressing any problems in service provision
A service coordinator compiles all documentation needed to establish eligibility, and in conjunction with the individual develops a Service Plan which describes his/her interests, strengths, and goals.
Ensuring annual service plans(RevisedServicePlans/RSPs)are completed in a timely manner

Ensuring each participant's records are maintained for at least six(6) years after termination of waiver services.
